Annual Performance Evaluation

In accordance with NL Health Services policy, an annual performance evaluation is required for all employees. Please complete the below questionnaire.

8. Do you maintain a positive working relationship with the office and use appropriate lines of communication to address any concerns\ incidents\ complaints.

9. Do you adhere to time schedules concerning arrivals and departures?

10. Do you attend work regular and notify the office in advance if you have an appointment and require time off?

11. Do you set priorities and schedule time effectively to complete tasks within established time frames?
